Transcript Request

If you are a former student of Serra High School, all transcript requests must be done by mail.

Please furnish the following information:

1) Name: (your name while attending Serra---Maiden, nickname, Jr. etc.)
2) Birthdate:
3) Year of Graduation:
4) Phone number where you can be reached
5) Self Adddressed Stamped Envelope
where you want official transcript mailed.
6) $1.00 per transcript in the form of cash, check or money order for curent students, $3.00for graduates.
Make check payable to “Serra High School”.
